- Certificate of All Historical Matters (issued within 3 months)

- Other documents deemed necessary by the secretariat
Self-employed(Blue Return) ：
- 2019, 2020 final tax return table 1 or e-mail at e-TAX or tax payment certificate 2 For income amount (in the case of a Blue Return, the business income amount is stated) or taxation certificate or tax exemption certificate
- January-March 2021 Sales Ledger
- Bankbook
- Identification card (driver’s license, individual number card, Juki card with photo, residence card)
- Affidavit / Consent
- Other documents deemed necessary by the secretariat
